What To Expect At Your Visit After you sign in our front office personnel will request: Insurance Card Photo ID Co-Pay (To speed up the check-in process , please click on the Registration Form, complete all the information, and bring the completed form in with you) A triage nurse will then call you and take vitals (Blood pressure, Pulse and Temperature).
